The application of alternative medicine, alone or in combination with conventional forms of treatment, is increasing in prevalence. Some of the more radical and untested forms of alternative medical therapy are practiced outside the United States. Nutritional and metabolic, pharmacologic, and immunologic therapies are available. Many of these treatments have not been adequately evaluated for safety and efficacy. The disregard of scientific knowledge and methods and the exclusive use of unconventional modes of prevention, diagnosis, and treatment of illness are often ill advised. Many forms of alternative medicine are available to patients today. "Alternative" as a word modifying "therapy, " "medicine, " or "treatment" refers to a variety of methods that lie outside the realm of conventional modern medical therapy.1 Some alternative practices originated in antiquity, whereas others have appeared more recently. A few have been successfully integrated with standard medical therapy; these approaches, known as "complementary therapies, " include nutritional interventions, herbal products, massage, and visualization and relaxation techniques, which are combined with conventional cancer therapies, such as surgery, radiation therapy, and chemotherapy.2 Certain dangers are inherent in the adoption of some alternative medical therapies, especially when they entail the abandonment of medically proven approaches in favor of unproven therapies.3, 4 The exclusive use of alternative therapies for diagnosing or treating life-threatening maladies, such as cancer, can be fatal. Aspirin may offer some protection but it is not as significant. The protective benefits of other NSAIDs, including naproxen Aleve, Naprosyn, Naprelan, Anaprox ; , and the newer COX-2 inhibitors also require research.

These drugs present challenges for oral delivery because they are often large molecules macromolecules ; which are inactivated in the gastrointestinal tract, have limited ability to cross cell membranes and generally cannot be delivered orally.

The benefits of pharmacologic lowering of blood pressure have been demonstrated in numerous randomized placeboor active-controlled trials of substantial size that used a variety of pharmacologic agents or classes of agents. Regardless of the pharmacologic mechanism by which blood pressure lowering occurs, the most consistent benefit demonstrated has been reduction of stroke and coronary heart disease. Table 3 provides a list of prospective, randomized, mostly placebo-controlled trials conducted over the last 3 to 4 decades that demonstrate the benefit of antihypertensive therapy. As shown in the table, a variety of pharmacologic agents have been represented, including diuretics, beta-adrenergic receptor blockers, angiotension converting enzyme ACE ; inhibitors, calcium channel blockers, and angiotensin receptor blockers.16-29 The fact that similar results have emerged from studies that used pharmacologically distinct agents provides strong evidence that it is the effect on blood pressure, not some other property of the drugs, that mediates the beneficial effects seen.
